How often do lipomas happen?

A lot of people have lipomas. About one person in every 1,000 has a lipoma. Lipomas can happen to both men and women.

Where can lipomas be found?

Lipomas are capable of developing anywhere on the body. Lipomas rarely develop on muscles, internal organs, or the brain. Although multiple lipomas can develop, the majority of individuals with lipomas have only one. The majority of lipomas develop just beneath the epidermis on:

  • Legs or arms.
  • Back.
  • Neck.
  • The shoulders.
  • Chest and torso (trunk).
  • The forehead.

Types of lipomas:

Based on the number of lipomas:

  • Solitary or single
  • Multiple

Solitary or single lipomas are less common in our practice. The age group of such patients is usually over 40 years. The most common areas are the neck, forehead, shoulder, back and chest. These lipomas are usually large in size (> 5cm). The causes are mostly unknown.

Multiple lipomas are more common in our practice. The age group of such patients is between 20 – 30 years. They typically notice small lumps in their early 20s which gradually grow in size until their 30s. The number of lipomas all over the body can be as low as 10 lipomas and can go as high as more than 200 lipomas in a single patient. Every lipoma is composed of fat. Some lipomas contain blood vessels and additional tissues. There are various types of lipomas based on components, which include:

  • Angiolipoma Lipoma contains adipose and blood vessels. Angiolipomas are typically excruciating.
  • The most prevalent form, a conventional lipoma contains white fat cells. White adipose tissue stores energy.
  • Fibrolipoma consists of fatty tissue and fibrous tissue.
  • Hibernoma lipoma is composed of brown fat. The majority of other lipomas are composed of white fat. Brown fat cells generate heat and aid in temperature regulation.
  • Myelolipoma: These lipomas contain adipose and blood-producing tissues.
  • Spindle cell lipomas contain fat cells that are longer than they are wide.
  • Pleomorphic lipomas contain fat cells of varied sizes and shapes.

What are the Causes of lipoma?

  • ultiple lipomas have a strong genetic connection. Most of these patients have a relative with lipomas.
  • Solitary lipomas’ causes are still uncertain.

There are certain conditions like:

  • Madelung’s disease in which men who drink heavily develop symmetric large lipomas in the upper body (shoulders, back and chest) giving the fake appearance of a muscular body.
  • Another condition called Dercum’s disease (also called Adiposis dolorosa)is common among obese women near menopausal age. The lipomas are multiple and painful which don’t respond to pain medication. The cause is unknown.
  • In Gardner’ssyndrome multiple lipomas can develop along with fibromas, desmoid tumors and epidermoid cysts. It is a hereditary condition with multi organ involvement.

Factors that could put you at risk of Lipoma

Medical experts don’t know what causes a lipoma, but some think it’s a response to a physical injury. Some people think that the lipoma was there before the injury and that the accident just brought it to light.

Here are some things that could put you at risk:

Not Exercising Enough:  Physical fitness may help in numerous ways, including providing protection. Lipomas, according to some medical professionals, tend to develop in sedentary persons.

Hereditary: It’s possible that your genes had a part in developing a lipoma.

Age: Even though lipomas can happen to anyone, they are most common in people between the ages of 40 and 60.

Certain health problems:  People with Gardner syndrome, which is a genetic illness that causes benign and dangerous tumors, adipose dolorosa, family multiple lipomatosis, or Madelung disease (most common in guys who drink a lot) may get one or more lipomas.

Symptoms of Lipoma

They are painless soft lumps underneath the skin, which move freely in all directions when pressure is applied to them. Very rarely they may be painful when they are compressing adjacent nerves, vessels or muscles.

Most lipomas don’t hurt, but they can be uncomfortable if they grow near a joint or press on a nerve. Many people with lipomas don’t feel anything. Most lipomas are:

  • They stay in one place and don’t spread to the tissues around them.
  • Some lipomas cause pain and soreness, based on where they are, how big they are, and whether or not they are near blood vessels.
  • Most of the time, the fatty lumps of hard tissue are round or oval in form.
  • Moveable: They are just under the surface of the skin and move when you touch them.
  • Some lipomas can be bigger than 6 inches across, but most are smaller than 2 inches.

How do doctors diagnose  lipomas?

Most of the time, a lipoma is found during a physical check-up. Your doctor or nurse will touch the lipoma and ask if it hurts. A sample may be needed to make sure that the lipoma isn’t cancer. During this process, your doctor will take a small piece of the lipoma and send it to a lab for tests.

People often think that these are cysts. Your doctor may ask for an imaging test like an ultrasound, magnetic resonance imaging (MRI), or computed tomography (CT) scan to get a clear picture of this lump. These imaging tests help your doctor figure out if it is a cyst or a lipoma.

It can also help find out where the lipoma is, how deep it is, if it has blood vessels, and if it is putting pressure on nerves or other organs. Most of the time, a sample isn’t needed. But your doctor may want to take a small piece of the lump to look at under a microscope.

The goal of these screening methods is to rule out liposarcoma, a rare cancer of fatty tissue. Liposarcomas are different from lipomas in that they are often painful, grow quickly, and cannot be moved with a finger.

Treatment options for lipoma:

There is no effective medicine for lipoma treatment. Injections such as steroids and Phosphatidylcholine have been used, but the results are not promising. Surgical excision remains the most effective method for lipoma treatment.

Lipoma Removal Procedure

After confirming the diagnosis, the patient is given a small set of blood tests. They include Viral serology, Blood sugars, Hemoglobin, and Coagulation profile. Lipoma removal surgery is done as an outpatient procedure and does not require admission.

This procedure is done under local anesthesia where an injection with a very fine needle is given to the skin over each lipoma separately to make it numb. Dr. Gautam Reddy being a Plastic Surgeon makes the smallest possible incisions, extracts the lipoma and closes it with the finest suture materials. Most of the time, Dr. Gautam Reddy uses suture-less techniques for the best cosmetic result with minimal scarring.

The patient is asked to come for follow-up 2 times after the procedure, to make sure that everything is fine.

Getting ready for lipoma surgery

Before removing a lipoma, your doctor will talk to you about how to get ready. Most of the time, these steps are:

  • Cleaning the area with soap that kills germs
  • Not shaving the area that will be treated
  • If you take any ACE blood pressure medicine skip that.
  • Not taking any blood thinners other than aspirin for a certain amount of time
  • Your doctor will give you specific advice about how to take your drugs, so be sure to tell him or her about all of them, as well as any extras you take. If you will be put to sleep for your treatment, you may need to stop eating and drinking for about 6 hours before.

Getting better after surgery

After your lipoma is removed, you may feel some pain in the area from where it was taken out for a few days. And if the cyst or lipoma is on your head, you may get headaches from stress. However, this is not very serious.

Most of the time, you can treat the symptoms with over-the-counter medicines. You can also use ice as often as your doctor tells you to. As your surgery wound heals, you should also stay away from hard exercise for about a week.

From there, it shouldn’t bother you anymore, so you should be able to go about your life as usual. And because we use methods that cause the least amount of damage, scars should be minor.

When should you talk to your doctor about lipomas?

See your doctor if you have a growth, lump, or knot under your skin. It’s important to get checked out and rule out major problems, like liposarcoma, which is a type of cancer. Some liposarcoma symptoms are the same as lipoma symptoms. If you have a lipoma and notice any changes, like sudden pain, you should see your doctor. Call your doctor if it grows quickly (in days or weeks), feels hard, or doesn’t move as easily. Your doctor will look at the lipoma to see if it has blood vessels, which is a sign of an angiolipoma, a rare and painful type of lipoma.

Before & After
FAQ`S
What is a Lipoma?

Lipoma is a benign tumor consisting of fat tissues. They are not considered dangerous.

Are they cancerous?

No. It is extremely rare. Few subtypes have more potential for cancer (e.g. Dysplastic, Atypical Spindle cell lipomas, pleomorphic lipomas, solitary fibrous lipomas, etc.)
Even among cancer-converted lipomas (Liposarcomas), 45% are locally invasive and not metastatic (Differentiated lipomas, Atypical lipomatous tumors). The remaining 55% (e.g. Dedifferentiated, myxoid, pleomorphic, etc.) are metastatic i.e. dangerous spread potential.

Types of lipomas?

Clinically:

  • By number Solitary, Multiple
  • By size: Small, medium, large

Histologically:

Adenolipoma, angiolipoma, angiolipo-leiomyoma, hibernoma, fibro lipoma, spindle cell lipoma, pleomorphic lipoma, superficial subcutaneous lipomas, etc.

Where can a lipoma be found in the body?

Lipomas originate from fat tissues present all over the body. Externally (below skin level), in the case of multiple lipoma patients, upper limbs, thighs, abdomen, flanks and back are common.

In the case of solitary lipomas, the common areas are the neck, face, back, shoulders, arms, and legs. But they can appear anywhere.Internally, they can appear in the brain, kidneys, intestines, etc.

What age group does lipoma affect?

Multiple lipomas start appearing in the early 20s and gradually grow. Most of them seek doctor’s help before their 30s. Solitary lipomas are common in people older than 40 years.

What causes lipoma?
  • Multiple lipomas have a strong genetic tendency.

There are certain conditions like:

  1. Madelung’s disease which is common in alcoholic men
  2. Dercum’s disease which is common in menopausal women.
  3. Gardner’s syndrome
  • Solitary lipoma causes are not well understood.
Clinical features of lipoma?

Soft, painless lumps of varying sizes that move in all directions under the skin when pressure is applied (slip sign).

How do we diagnose lipomas?

Diagnosis is mostly made when an experienced surgeon examines the patient clinically in the OPD. Sometimes, when in doubt, an imaging technique might be advised by the surgeon. The gold standard of imaging in lipomas is MRI (it can differentiate between lipoma and liposarcoma). CT scans and Ultrasound scans are also useful.
FNAC is not of much use. It is done only when a cancer is suspected which is extremely rare.

What are the Treatment options for lipoma?
  • Surgical excision is the gold standard of treatment for lipoma. Complete excision is possible only in excision, which also prevents recurrence.Liposuction which is also a surgical procedure is reserved for those patients who have multiple (too many) large soft lipomas in the trunk or thighs. It is not possible to remove lipomas completely by liposuction and hence recurrence rates are high.Targeted cauterization is an alternative method, with low success and high recurrence.Non-surgical techniques like steroid injections and phosphatidylcholine (PPC) injections have poor results. There is no Tablet or ointment medication that is approved for lipoma treatment.
Will lipomas come back after the treatment?

Surgical excision has no recurrence but new lipomas can grow nearby or in other areas of the body.

How do I know if the lipoma is turning into cancer?
  • The following are bad signs for a lipoma:Rapidly growing, painful, skin over the swelling is not pinchable, ulcers form over the swelling, lump is fixed to the muscles below.

Can I prevent lipomas?

Preventing lipomas is not always straightforward, but there are steps you can take to potentially lower your risk of developing these benign fatty tumors. Maintaining a healthy weight through a balanced diet and regular exercise is a key factor, as lipomas are more common in overweight individuals. Consuming a diet rich in fruits, vegetables, and whole grains while limiting processed foods and excessive dietary fat may also help reduce the risk. Staying hydrated, exercising regularly, and avoiding excessive alcohol consumption and tobacco use can contribute to overall health and potentially reduce the likelihood of lipoma development. Managing stress through techniques like meditation and protecting your skin from excessive sun exposure are additional measures to consider. While these steps may help lower the risk, it’s important to remember that genetics can play a role, and lipomas can run in families. If you notice any new lumps or growths under your skin, consult a healthcare professional for proper evaluation and guidance, as early detection and diagnosis are essential for appropriate management.
